Dear Parents,

The Blair Athletic Booster Club would like to post pictures of Blair athletes from past, present and future sporting events that would be on our website and possibly basketball programs. To ensure the safety of your child, first names will be the only means of identification posted on the website. Furthermore, if we do not have a release for your child and a group picture is used, your child's name will not be listed. If you do not object to having your child's picture used on Blair Athletic Booster Club publications, please sign and date the release below and have your child return the signed form to their coach or to any Blair Athletic Booster Club officer.
